Our Support Coordination and Care Management services help individuals and families navigate complex healthcare and social service systems with confidence and clarity. Our experienced coordinators work closely with individuals, caregivers, healthcare providers, and community partners to ensure access to the services and resources necessary to meet each person’s unique needs. Support coordinators assist with developing and implementing Individualized Service Plans (ISPs) that reflect the individual’s goals, strengths, and personal preferences. They also monitor services, advocate for necessary supports, and ensure that individuals receive high-quality, coordinated care. By acting as guides and advocates, our coordinators empower individuals and families to make informed decisions while promoting independence and self-determination.
